I understand that I must be able to perform the essential functions of any job for which I am offered employment, which might include physical standards established by CMA. I understand that if offered employment, I might be required as a condition to employment to take a physical examination. I also understand that during employment, I might from time to time be subjected to physical examination and/or physical tests to demonstrate that I can perform the essential functions of my job.
I understand that CMA my from time to time require that I take a drug and alcohol test as a condition of employment.
I hereby give consent to any previous employer to provide employment-related information about me to CMA and will hold CMA, and any former employee harmless from every claim made on the basis that such information was provided or that any employment decision was based on that information.
I hereby authorize CMA to contact a company or person I listed in the Education, Work Experience, and/or References section of my employment application. Furthermore, I authorize said company, person, or educational institution to disclose certain information regarding my employment history and/or qualifications for employment, mainly my dates of employment, the descriptions of the jobs I performed, and the compensation I received.
